In northamptonshire, 75 in every 100,000 people die prematurely before the age of 75 from cardiovascular disease. An it solution is required to support the operational process and monitor the performance and effectiveness of the nhs health check programme at all levels. The purpose of the it support system is to enable health check providers to effectively carry out the nhs health check programme and for local authority commissioners to effectively manage and quality assure the programmes processes and outcomes. The service will be required from 1st april 2019 for an initial period of 1 year (up to 31st march 2020) with an option to extend for a further period of up to 2 years, taking the total contract term to a maximum of 3 years. Any extension will be agreed between the council and the provider.
