Said prospective suppliers are required to submit the following documents together with the quotation/written proposals which shall be evaluated on June 26, 2024, 9:00 o’clock in the morning at the Municipal Hall, Conference Room, Municipality of Sta. Rita, to wit: 1. Valid and current Mayors Permit 2. DTI Registration/SEC Registration 3. PhilGEPS Registration 4. BIR Registration 5. Omnibus Sworn Statement The criteria to be used for the eligibility check of the prospective bidders, examination and evaluation of bids, post-qualification and all matters relevant to this procurement shall be in accordance with Republic Act. No. 9184 (The Government Procurement Reform Act) and its Implementing Rules and Regulations. Interested Bidders may obtain further information from Municipality of Sta. Rita at the address given below from Monday to Friday at 8:00 am to 5:00 pm Quotation/proposal fee of Five Hundred Pesos (Php500.00) - Non Refundable. The Municipality of Sta.
