Eligible consultants of known qualifications are hereby invited to submit their proposals, signed by the duly authorized representative, on or before the deadline indicated above, subject to the Terms of Reference (TOR) attached as Annex “A” and the Terms and Conditions (TOC) provided in this RFP. Proposals may be submitted physically or via email through the contact information indicated below: Procurement Planning and Management Division Department of Information and Communications Technology C.P. Garcia Ave., Diliman, Quezon City Email: ps.ppmd@dict.gov.ph Please submit your proposal along with the following mandatory requirements marked with “X”: X Latest Business/Mayor’s Permit issued by the city or municipality where the principal place of business of the bidder is located; X Curriculum Vitae (CV) or Professional License of each team member; X Valid PhilGEPS Registration Number/Organization ID/PhilGEPS Certificate of Registration (Platinum Membership); X Latest Income or Business Tax Return filed and paid through the Electronic Filing and Payment System (for ABCs above ₱500,000.00); and X Notarized Omnibus Sworn Statement using GPPB-Prescribed Format, accompanied with proof of authorization, if applicable (for ABCs above ₱50,000.00). Proof of authorization shall be a duly notarized Secretary’s Certificate, Board/Partnership Resolution, or Special Power of Attorney, whichever is applicable, in case of a corporation, partnership, or cooperative, or joint venture; or a duly notarized Special Power of Attorney in case of a sole proprietorship, giving full power and authority to its officer to sign the OSS and do acts to represent the Bidder. In addition, prospective Consultants are requested to submit the following documents for the evaluation of their technical proposal. Nonetheless, non-submission of any of the documents is not a ground for disqualification of the technical proposal provided that minimum mandatory technical requirements, if any, and a rating of at least 70% for the technical proposal are complied with: List of completed similar projects of the Firm for the last 5 years accompanied with documentary proofs, if any. Similar Projects shall refer to contracts with a scope of work related to providing general structural engineering design, assessment and investigation including retrofitting for diverse project classification and usage; Valid professional License of each team member, if applicable; List or CV containing the Related or Relevant Trainings indicating among others the number of hours attended and completed of each Team Member, with documentary proofs, if any. Related or relevant trainings shall refer to general engineering design, structural assessment and investigation, and/ or engineering cost estimate (see requirement for each personnel); and Plan of Approach and Methodology.
