Background and Rationale: In line with the Executive Order \ 605, dated February 23, 2007, otherwise known as “Institutionalizing the structure, mechanisms and standards to implement the Government Quality Management Program (GQMP), amending for the purpose of Administrative Order no. 161, s 2006” directed all departments and agencies of the executive branch to adopt the ISO 9001:2008 Quality Management System as part of the implementation of a government wide quality management program. The Tondo Medical Center advocates the need to be a Certified ISO 9001:2015, with an objective of continuous improvement that aims for the satisfaction of clients and customers. The Tondo Medical Center in an effort to serve effectively and efficiently the customers and meet their satisfaction provides services and processes with consistent improvement, acquiring high quality, and well maintained equipment and facilities, thus the need for Preventive and Corrective Maintenance Service is necessary. II. Objectives: To maintain the waste water treatment plant in functional condition based on the acceptable standards. Specific Objectives 1. To implement a preventive maintenance program to help prevent and correct deficiencies in the different components of the Waste Water Treatment Plant minimizing breakdown and service interruptions, extending component service life and maximizing operating efficiency. 2. To include periodic inspections and services as indicated in the preventive maintenance scope of works and procedures to be conducted once every month. The PM program is established based on manufacturer’s recommendations, handbooks, operating/service manuals, contractor expertise and general engineering judgments. 3. To submit a preventive maintenance plan which shall include equipment components to be inspected and maintained, inspection and maintenance techniques, inspection and maintenance frequencies and reporting methodology. 4. To perform, manage and monitor the planned preventive maintenance services to ensure that the wastewater treatment plant services are fully functional at Tondo Medical Center in accordance with the scope listed in the scope of works. 5. To provide preventive maintenance activities, including equipment performance assessment and checks to detect incipient faults, cleaning and lubrication as needed. The Engineer/technician will monitor equipment deterioration informing the hospital on repair or replacement of worn-out parts as needed. 6. In case of a breakdown, to carry out plant assessment and recommend corrective action subject to the specific instructions from Tondo Medical Center Engineering and Facilities Management Section. III. SCOPE OF WORK: 1. Perform planned preventive maintenance service of the Waste Water Treatment Plant. The preventive maintenance activities shall include planned guidelines of activities and equipment diagnostics and other related works: lubrication, oiling, greasing and cleaning. 2. During the scheduled PMS, report to the Engineering and Facilities Management Section which part is showing signs of wear and tear or the cause of breakdown. 3. Ensure that the replacement parts which is chargeable to client are genuine and that the service team is trained and knowledgeable in replacing defective spare parts. 4. Service reports should be forwarded to the following: a. Engineering Facilities Management Section b. Head of Material Management Section Annex A Scope of Works of planned preventive maintenance Plant Exterior Description of Works: The external portion of the plant is inspected. Starting with the opening of manholes (main connected to the pumpstation) to check if water coming from the different areas in the hospital freely flows into the pump station. The preventive maintenance provider will do cleaning works on top stabs of the pump station and buffer tanks and also the exposed portion of the pipes. Pump Station Description of Works: Manhole covers are removed to be able to check for possible accumulation of mud, dirt or other waste in the sump pit. It is the preventive maintenance provider responsibility to keep this clean and free of dirt. The pump will then be inspected for defects and have the oil checked and /or replaced if needed. Float sensors are inspected for defects and to be cleaned free from dirt for better and effective operation. Procedure: 1. Check oil level. Change the oil every year or when the oil is no longer transparent. 2. Inspect cable entry. 3. Inspect for leakage especially when pump is in use for a long time. 4. Inspect the motor housing. 5. Unscrew the inspection plug of the motor housing. 6. Inspect plug if water is present due to condensation. 7. Inspect for corrosion. 8. Check for accumulated solids or waste in the sump pit. 9. Check condition of float sensor. Thoroughly clean for better operation. 10. Check terminal/connection box. 11. Check rubber gasket. Roto-sieve Description of Works: The roto-sieve cover is remove to expose the rotating sieve drum and other components for wear and tear, such as trunnion wheels, collection through, outlet, drive system, roller brush, spray header with nozzle, brush bearing, conductive electrodes and terminal box. Procedure: 1. Inspect screen drum (rotating sieve drum) for damages or deterioration. 2. Flush sieve drum inside and outside. 3. Inspect rotating brush. Clean and adjust properly. 4. Inspect brush bearing, Apply WD40 and/or grease as needed. 5. Clean spray nozzle. 6. Inspect water /collection trough. 7. Inspect collection hopper clean if necessary. 8. Flush screen discharge. 9. Inspect conductive electrode, making sure that it’s functioning properly. 10. Check water supply. Roto-Press Description of Works: Roto-press connection flange is disconnected to completely clean the roto-press feed pipe and piston. After which, conduct maintenance on piston press motor together with the unit micro switches. Procedure: 1. Check the function of the microswitches manually. 2. Clean slot openings. 3. Check the bearing for the guide pin if it is rotating. If not conduct cleaning or repair if needed. Apply grease for better rolling action. 4. Check electrical leads or connection for wear and deterioration. 5. Rinse water through. 6. Check amperage. 7. Check electrical connections. 8. Check bearing and lubricate the same. Microfilter Description of Works: The cover of the microfilter is carefully removed to expose the filter drum and other concealed parts. Filtering elements are carefully cleaned to maximize filtering capacity of the machine. Procedure: 1. Inspect spray nozzle. Change spray nozzle on the first PMS with one year warranty. Clean nozzle with soft brush or compressed air every PMS. 2. Check the level sensor. Clean properly. 3. Inspect ball bearing. Clean and apply grease. 4. Check filter elements. Clean properly to open up clogs. 5. Check gear motor. 6. Apply grease on chain. 7. Inspect rubber seals for deterioration or defects. 8. Regulate the chain tension. 9. Conduct maintenance on the progressive cavity pump. Rinse Water Station Description of Works: The water station will be checked for leaks and will be cleaned properly. Procedure: 1. Check function control. 2. Check filter (suction/pressure sides.) 3. Inspect pipe connection for leaks. Blow Reactor Description of Works: The aerators are detached from its mounting platform on top of reactor tank and dismantled to clean the interior of the motor thoroughly removing the old and worn-out carbon vanes, residues, filters and gaskets carefully replacing them with new sets of spare parts. It will then be brought back to its mounting platform for reinstallation. Procedure: 1. Check control valves for tightness. 2. Disassemble blower motor. 3. Replace old carbon vanes. 4. Replace new filter cartridges (big/small). 5. Check bearing. Apply grease. 6. Replace new bearing every year or as needed. 7. Check the rubber coupling for deterioration or defect. 8. Check the cooling fins and cooling pipes for clogging. 9. Clean cooling fins and cooling pipes using high pressure air. Reactor Tanks Description of Works: Reactor tanks will be checked for leaks and cracks. Sludge will be extracted out to clean the interior of the tank while the exterior face be cleaned too. Procedure 1. Cleaning of exterior part of wall. 2. Cleaning of pipes/pressure switch. 3. Check tightness of fitting and connections. 4. Check water surface level/level during aeration (visual check). Check for leaks. 5. Rinse pipes (over flow, ventilation.) 6. Check the o-ring on the reactor tank. Air Compressor Description of Works: Cut-off and switch on pressures are checked and adjusted if necessary. Condensate water is also taken off. Oil level is checked and replenished if needed. Procedure: 1. Check cable connection for deterioration. 2. Check pressure gauge. Adjust if necessary. 3. Check oil level. 4. Change/replenish oil if necessary. 5. Check for condensate. 6. Drain off water condensate from air receiver. 7. Check intake filter. 8. Clean filter. Replace every year. Line Pipes Description of Works: Line pipes will be checked for deterioration and leaks. Procedure: 1. Check for tightness. 2. Check for Leakage. IV. DELIVERABLES: The Service Provider shall provide the following deliverables: 1) Monthly Preventive Maintenance visit (PMS) for four months, PMS report within 1 working day after every conduct of PMS. 2) Pass monthly water analysis test based on PSIC 86 and 87 and supply of 15 Kgs. of chlorine monthly. 3) One time replacement of the following materials (consumables) 7 days upon Receipt of Notice to Proceed. Carbon Vanes- 4 pcs/set (3 sets) Filters (1 big/2 small) (3 sets) Gaskets (3 different size /set) V. PAYMENT SCHEDULE: Payment shall be paid in full upon completion of 4 months preventive maintenance and completion of deliverables. VI. PENALTY: 1. In case of failure to fulfill deliverables for the monthly water analysis, PMS, and carbon vanes, filters, gaskets and chlorine, a penalty of one tenth (1/10) of one percent for every day of delay shall be imposed. 2. In case the total sum of liquidated damages reaches ten percent (10%) of the total contract price, the Procuring Entity concerned may rescind the contract and impose appropriate action. VII. IMPLEMENTING ARRANGEMENTS: 1. The following shall be the technical and administrative office responsible for the project. Responsibilities of the Service Provider • Monthly Monitoring and Preventive Maintenance in accordance to Annex A Scope of Work including greasing, oiling, and Cleaning to include float switch, level switch/limit switch and chlorine dosing pump. To submit monthly monitoring and maintenance activity report supported by picture evidence. • Provide unlimited on-call corrective maintenance service as required within 24 hours response time from notice or receipt of call with proper coordination with TMC EFMS and thus be conducted during office hour. In case of parts replacement, only parts will be billed and no labor cost at all. • The Service Provider shall submit/inform the Engineering Facilities Management Section or the Material and Management Section of a schedule preventive maintenance activity. • The Service Provider shall coordinate with the Engineering Facilities Management Section or the Material and Management Section regarding any changes on the date of schedule of deliverables or any delay in the activities related to this. • Report to the Engineering and Facilities Management and to the End-User which part is showing signs of wear and tear that will cause breakdown within 1 working day. • Monthly Water Analysis every 1st week of the month to be submitted to EFMS and supply of 15 Kgs chlorine powder monthly. (Testing and retesting shall be shouldered by the service provider until all parameters meet the standard of DENR & LLDA). • The Service Provider is responsible for the safety and damage of the machine resulting from its faulty workmanship and negligence. • In the event the Waste Water Treatment Plant damage is not covered by the scope of this agreement, the Service Provider representative must show the physical evidence of the cause of damage with the Engineering Facilities Management Section of Tondo Medical Center. • The Service Provider must ensure the functionality of the Waste Water Treatment Plant and the compliance to standards of water analysis result and other deliverables. • Service Provider shall be liable to any violations or penalty imposed by DENR/LLDA/City Ordinance or any government regulating body resulting from breakdown or poor performance of waste water treatment facilities or failure of water analysis sampling. Monthly deliverables of laboratory testing water quality analysis using PSIC 86 & 87 to be submitted every 1st week of the month with the following significant Parameters as follows: Color, Temperature, pH, BOD, Total Suspended Solids, Fecal Coliform, Ammonia, Nitrate, Phosphate, Oil and Grease, Surfactants. 3) Responsibilities of Tondo Medical Center: • The Tondo Medical Center through the Engineering and Facilities Management Section and Material and Management Section, shall closely coordinate with the Service Provider in the conduct of preventive maintenance and other related activities. • The Tondo Medical Center Engineering Department shall support the conduct of preventive maintenance ensuring that any support with regards to access to WWTP and other utilities will be extended to the preventive maintenance provider. VIII. PROJECT ARRANGEMENT/CONTRACT MANAGEMENT: 1. The project will be handled by a team composed of a representative of the Material and Management Section, Engineering and Facilities Management Section and Technical Working Group, Head of Bids and Award Committee (BAC) under the supervision of the Chief Administrative Officer. The said team is also assigned to handle Preventive Maintenance Management. 2. The representative of the Maintenance Service Provider shall submit the deliverables and other reports to the office of the Engineering and Facilities Management Section and Material and Management Section for validation. In case there are additions or changes that should be incorporated into the submitted deliverables, the Material and Management Section Head shall notify the representative of the Service Provider. 3. The Engineering and Facilities Management Section shall issue a Certificate of Accomplishment in the report submitted by the Service Provider with the approval of Medical Center Chief and facilitate documentation for payment. IX. Reporting Scheme: The Service Provider shall coordinate closely with the Engineering Department and Material and Management Section to clarify on the scope of activities and other matters related to the preventive maintenance of WWTP. X. Sustainability Factor: Qualification of the Waste Water Treatment Plant Maintenance Service Provider Company/Firm The WWTP Maintenance Service Provider should have adequate technical and management capability in conducting preventive and corrective maintenance of Waste Water Treatment Plant, Sequential Batch Reactor (SBR) type. • The company has at least 10 years of experience maintaining WWTP of Sequential Batch Reactor type. To present proof of length of experience. • The company has very satisfactory performance from present or previous client with proof of certification. • Positive feedback from previous clients especially on delivering services and deliverables on time as per contract compliance. Negative feedback or undesirable comment with proof of evidence is enough ground for disqualification. Key Personnel The key personnel of the maintenance service provider shall have at least (5) years Experience in working with the same SBR type Waste Water Treatment Plant with valid training Certificate for the same stated equipment. Atleast one (1) Lead Engineer- Licensed BSME OR BSEE w/ at least 5 years experience in maintenance related works with SBR type Waste Water Treatment Plant. Atleast two (2) Technician - Completion of mechanical, electrical or electronic related Technical Course with at least 5 year experience in working with SBR Type Waste Water Treatment Plant. XIII. Background and Rationale: In line with the Executive Order \ 605, dated February 23, 2007, otherwise known as “Institutionalizing the structure, mechanisms and standards to implement the Government Quality Management Program (GQMP), amending for the purpose of Administrative Order no. 161, s 2006” directed all departments and agencies of the executive branch to adopt the ISO 9001:2008 Quality Management System as part of the implementation of a government wide quality management program. The Tondo Medical Center advocates the need to be a Certified ISO 9001:2015, with an objective of continuous improvement that aims for the satisfaction of clients and customers. The Tondo Medical Center in an effort to serve effectively and efficiently the customers and meet their satisfaction provides services and processes with consistent improvement, acquiring high quality, and well maintained equipment and facilities, thus the need for Preventive and Corrective Maintenance Service is necessary. II. Objectives: To maintain the waste water treatment plant in functional condition based on the acceptable standards. Specific Objectives 1. To implement a preventive maintenance program to help prevent and correct deficiencies in the different components of the Waste Water Treatment Plant minimizing breakdown and service interruptions, extending component service life and maximizing operating efficiency. 2. To include periodic inspections and services as indicated in the preventive maintenance scope of works and procedures to be conducted once every month. The PM program is established based on manufacturer’s recommendations, handbooks, operating/service manuals, contractor expertise and general engineering judgments. 3. To submit a preventive maintenance plan which shall include equipment components to be inspected and maintained, inspection and maintenance techniques, inspection and maintenance frequencies and reporting methodology. 4. To perform, manage and monitor the planned preventive maintenance services to ensure that the wastewater treatment plant services are fully functional at Tondo Medical Center in accordance with the scope listed in the scope of works. 5. To provide preventive maintenance activities, including equipment performance assessment and checks to detect incipient faults, cleaning and lubrication as needed. The Engineer/technician will monitor equipment deterioration informing the hospital on repair or replacement of worn-out parts as needed. 6. In case of a breakdown, to carry out plant assessment and recommend corrective action subject to the specific instructions from Tondo Medical Center Engineering and Facilities Management Section. III. SCOPE OF WORK: 1. Perform planned preventive maintenance service of the Waste Water Treatment Plant. The preventive maintenance activities shall include planned guidelines of activities and equipment diagnostics and other related works: lubrication, oiling, greasing and cleaning. 2. During the scheduled PMS, report to the Engineering and Facilities Management Section which part is showing signs of wear and tear or the cause of breakdown. 3. Ensure that the replacement parts which is chargeable to client are genuine and that the service team is trained and knowledgeable in replacing defective spare parts. 4. Service reports should be forwarded to the following: a. Engineering Facilities Management Section b. Head of Material Management Section Annex A Scope of Works of planned preventive maintenance Plant Exterior Description of Works: The external portion of the plant is inspected. Starting with the opening of manholes (main connected to the pumpstation) to check if water coming from the different areas in the hospital freely flows into the pump station. The preventive maintenance provider will do cleaning works on top stabs of the pump station and buffer tanks and also the exposed portion of the pipes. Pump Station Description of Works: Manhole covers are removed to be able to check for possible accumulation of mud, dirt or other waste in the sump pit. It is the preventive maintenance provider responsibility to keep this clean and free of dirt. The pump will then be inspected for defects and have the oil checked and /or replaced if needed. Float sensors are inspected for defects and to be cleaned free from dirt for better and effective operation. Procedure: 1. Check oil level. Change the oil every year or when the oil is no longer transparent. 2. Inspect cable entry. 3. Inspect for leakage especially when pump is in use for a long time. 4. Inspect the motor housing. 5. Unscrew the inspection plug of the motor housing. 6. Inspect plug if water is present due to condensation. 7. Inspect for corrosion. 8. Check for accumulated solids or waste in the sump pit. 9. Check condition of float sensor. Thoroughly clean for better operation. 10. Check terminal/connection box. 11. Check rubber gasket. Roto-sieve Description of Works: The roto-sieve cover is remove to expose the rotating sieve drum and other components for wear and tear, such as trunnion wheels, collection through, outlet, drive system, roller brush, spray header with nozzle, brush bearing, conductive electrodes and terminal box. Procedure: 1. Inspect screen drum (rotating sieve drum) for damages or deterioration. 2. Flush sieve drum inside and outside. 3. Inspect rotating brush. Clean and adjust properly. 4. Inspect brush bearing, Apply WD40 and/or grease as needed. 5. Clean spray nozzle. 6. Inspect water /collection trough. 7. Inspect collection hopper clean if necessary. 8. Flush screen discharge. 9. Inspect conductive electrode, making sure that it’s functioning properly. 10. Check water supply. Roto-Press Description of Works: Roto-press connection flange is disconnected to completely clean the roto-press feed pipe and piston. After which, conduct maintenance on piston press motor together with the unit micro switches. Procedure: 1. Check the function of the microswitches manually. 2. Clean slot openings. 3. Check the bearing for the guide pin if it is rotating. If not conduct cleaning or repair if needed. Apply grease for better rolling action. 4. Check electrical leads or connection for wear and deterioration. 5. Rinse water through. 6. Check amperage. 7. Check electrical connections. 8. Check bearing and lubricate the same. Microfilter Description of Works: The cover of the microfilter is carefully removed to expose the filter drum and other concealed parts. Filtering elements are carefully cleaned to maximize filtering capacity of the machine. Procedure: 1. Inspect spray nozzle. Change spray nozzle on the first PMS with one year warranty. Clean nozzle with soft brush or compressed air every PMS. 2. Check the level sensor. Clean properly. 3. Inspect ball bearing. Clean and apply grease. 4. Check filter elements. Clean properly to open up clogs. 5. Check gear motor. 6. Apply grease on chain. 7. Inspect rubber seals for deterioration or defects. 8. Regulate the chain tension. 9. Conduct maintenance on the progressive cavity pump. Rinse Water Station Description of Works: The water station will be checked for leaks and will be cleaned properly. Procedure: 1. Check function control. 2. Check filter (suction/pressure sides.) 3. Inspect pipe connection for leaks. Blow Reactor Description of Works: The aerators are detached from its mounting platform on top of reactor tank and dismantled to clean the interior of the motor thoroughly removing the old and worn-out carbon vanes, residues, filters and gaskets carefully replacing them with new sets of spare parts. It will then be brought back to its mounting platform for reinstallation. Procedure: 1. Check control valves for tightness. 2. Disassemble blower motor. 3. Replace old carbon vanes. 4. Replace new filter cartridges (big/small). 5. Check bearing. Apply grease. 6. Replace new bearing every year or as needed. 7. Check the rubber coupling for deterioration or defect. 8. Check the cooling fins and cooling pipes for clogging. 9. Clean cooling fins and cooling pipes using high pressure air. Reactor Tanks Description of Works: Reactor tanks will be checked for leaks and cracks. Sludge will be extracted out to clean the interior of the tank while the exterior face be cleaned too. Procedure 1. Cleaning of exterior part of wall. 2. Cleaning of pipes/pressure switch. 3. Check tightness of fitting and connections. 4. Check water surface level/level during aeration (visual check). Check for leaks. 5. Rinse pipes (over flow, ventilation.) 6. Check the o-ring on the reactor tank. Air Compressor Description of Works: Cut-off and switch on pressures are checked and adjusted if necessary. Condensate water is also taken off. Oil level is checked and replenished if needed. Procedure: 1. Check cable connection for deterioration. 2. Check pressure gauge. Adjust if necessary. 3. Check oil level. 4. Change/replenish oil if necessary. 5. Check for condensate. 6. Drain off water condensate from air receiver. 7. Check intake filter. 8. Clean filter. Replace every year. Line Pipes Description of Works: Line pipes will be checked for deterioration and leaks. Procedure: 1. Check for tightness. 2. Check for Leakage. IV. DELIVERABLES: The Service Provider shall provide the following deliverables: 1) Monthly Preventive Maintenance visit (PMS) for four months, PMS report within 1 working day after every conduct of PMS. 2) Pass monthly water analysis test based on PSIC 86 and 87 and supply of 15 Kgs. of chlorine monthly. 3) One time replacement of the following materials (consumables) 7 days upon Receipt of Notice to Proceed. Carbon Vanes- 4 pcs/set (3 sets) Filters (1 big/2 small) (3 sets) Gaskets (3 different size /set) V. PAYMENT SCHEDULE: Payment shall be paid in full upon completion of 4 months preventive maintenance and completion of deliverables. VI. PENALTY: 1. In case of failure to fulfill deliverables for the monthly water analysis, PMS, and carbon vanes, filters, gaskets and chlorine, a penalty of one tenth (1/10) of one percent for every day of delay shall be imposed. 2. In case the total sum of liquidated damages reaches ten percent (10%) of the total contract price, the Procuring Entity concerned may rescind the contract and impose appropriate action. VII. IMPLEMENTING ARRANGEMENTS: 1. The following shall be the technical and administrative office responsible for the project. Responsibilities of the Service Provider • Monthly Monitoring and Preventive Maintenance in accordance to Annex A Scope of Work including greasing, oiling, and Cleaning to include float switch, level switch/limit switch and chlorine dosing pump. To submit monthly monitoring and maintenance activity report supported by picture evidence. • Provide unlimited on-call corrective maintenance service as required within 24 hours response time from notice or receipt of call with proper coordination with TMC EFMS and thus be conducted during office hour. In case of parts replacement, only parts will be billed and no labor cost at all. • The Service Provider shall submit/inform the Engineering Facilities Management Section or the Material and Management Section of a schedule preventive maintenance activity. • The Service Provider shall coordinate with the Engineering Facilities Management Section or the Material and Management Section regarding any changes on the date of schedule of deliverables or any delay in the activities related to this. • Report to the Engineering and Facilities Management and to the End-User which part is showing signs of wear and tear that will cause breakdown within 1 working day. • Monthly Water Analysis every 1st week of the month to be submitted to EFMS and supply of 15 Kgs chlorine powder monthly. (Testing and retesting shall be shouldered by the service provider until all parameters meet the standard of DENR & LLDA). • The Service Provider is responsible for the safety and damage of the machine resulting from its faulty workmanship and negligence. • In the event the Waste Water Treatment Plant damage is not covered by the scope of this agreement, the Service Provider representative must show the physical evidence of the cause of damage with the Engineering Facilities Management Section of Tondo Medical Center. • The Service Provider must ensure the functionality of the Waste Water Treatment Plant and the compliance to standards of water analysis result and other deliverables. • Service Provider shall be liable to any violations or penalty imposed by DENR/LLDA/City Ordinance or any government regulating body resulting from breakdown or poor performance of waste water treatment facilities or failure of water analysis sampling. Monthly deliverables of laboratory testing water quality analysis using PSIC 86 & 87 to be submitted every 1st week of the month with the following significant Parameters as follows: Color, Temperature, pH, BOD, Total Suspended Solids, Fecal Coliform, Ammonia, Nitrate, Phosphate, Oil and Grease, Surfactants. 3) Responsibilities of Tondo Medical Center: • The Tondo Medical Center through the Engineering and Facilities Management Section and Material and Management Section, shall closely coordinate with the Service Provider in the conduct of preventive maintenance and other related activities. • The Tondo Medical Center Engineering Department shall support the conduct of preventive maintenance ensuring that any support with regards to access to WWTP and other utilities will be extended to the preventive maintenance provider. VIII. PROJECT ARRANGEMENT/CONTRACT MANAGEMENT: 1. The project will be handled by a team composed of a representative of the Material and Management Section, Engineering and Facilities Management Section and Technical Working Group, Head of Bids and Award Committee (BAC) under the supervision of the Chief Administrative Officer. To present proof of length of experience. • The company has very satisfactory performance from present or previous client with proof of certification. • Positive feedback from previous clients especially on delivering services and deliverables on time as per contract compliance.
