Submit your quotation duly signed by your authorized representative not later than October 25, 2024 at 1:00 PM at the BAC Office, Dacanay Hall, City of San Fernando, La Union. The following documents shall be submitted on or before the deadline submission and shall conform with the following instructions: 1. All pages in the original copy shall be duly signed by the bidder or authorized representative. 2. All copies shall be properly tabbed, to wit: Tabbing Document Financial Documents A Original of duly signed and accomplished Financial Bid Form B Original of duly signed Bid Prices in the Bill of Quantities C Duly accomplished Detailed Estimates Form Legal Documents D Valid PhilGEPS Registration Certificate (PhilGEPS Registration Number) E Valid and Current Mayor’s or Business permit F Valid and Current Philippine Contractors Accreditation Board (PCAB) License G Income/Business Tax Return (for Projects with ABCs of P500,000.00 above) H Omnibus Sworn Statement (GPPB-prescribed forms) Technical Documents I Project Requirements, which shall include the following: I.a Organizational chart for the contract to be bid; I.b List of contractor’s key personnel (e.g., Project Manager, Project Engineers, Materials Engineers, and Foremen), to be assigned to the contract to be bid, with their complete qualification and experience data; I.c List of contractor’s major equipment units, which are owned, leased, and/or under purchase agreements, supported by proof of ownership or certification of availability of equipment from the equipment lessor/vendor for the duration of the project, as the case may be; J Original copy of Bid Security.
