LAGAO, GENERAL SANTOS CITY REFERENCE NO: NHAXII-2024-135 1) The National Housing Authority (NHA) – Region XII through its Regional Office Bids and Awards Committee (ROBAC), invites two (2) PhilGEPS registered and professionally regulated External Real Estate Appraisal Service Providers (EREASPs) to undertake the appraisal of developed land and completed housing units of Sofia Ville (AFP/PNP Housing Project) located in Nursery Road, Brgy. Lagao, General Santos City in accordance with Section 53.9 (Negotiated Procurement – Small Value Procurement) of the 2016 Revised Implementing Rules and Regulations of Republic Act 9184. Details of the project scope are specified in the Terms of Reference (TOR), attached hereto as Annex A. 2) The Approved Budget for the Contract (ABC) is Php200,000.00 (or Php 100,000.00 each), inclusive of all applicable government taxes and charges, professional fees, and other incidental and administrative costs. The engagement shall be for forty-five (45) Calendar Days commencing from the date of receipt of the Notice to Proceed (NTP) by the Consultant. Proposal received in excess of the ABC shall be automatically rejected during the opening of proposals. 3) Interested Bidders are required to submit in sealed envelopes, one (1) original copy and three (3) copies of the following to the ROBAC on or before October 30, 2024, 10:00 AM, at the NHA-Region XII Office, Prime Regional Center, Brgy. Carpenter Hill, City of Koronadal, South Cotabato. Late submissions shall not be accepted. A. Eligibility Documents: 1) Mayor’s/Business Permit; 2) PhilGEPS Registration Number/Certificate; 3) Omnibus Sworn Statement using GPPB prescribed Form (please use the form attached hereto as Annex B); The Omnibus Sworn Statement shall be accompanied by a duly notarized Special Power of Attorney, Board/Partnership Resolution, or Secretary’s Certificate, whichever is applicable, granting the representative of the bidder authority to do, execute and perform any and all acts necessary to participate, submit the bid, and to sign and execute the ensuing contract for the subject procurement. B. Technical Documents: 4) Curriculum Vitae (please use the form attached hereto as Annex C); 5) Consultant’s references providing completed projects in the last 5 years illustrating the relevant experience of the Consultant (please use the forms attached hereto as Annex D); 6) A concise, complete, and logical description of how the Consultant shall carry out the services to meet all requirements of the TOR (please use the form attached hereto as Annex E); 7) A work plan showing in graphical format (bar chart) the timing of major activities, anticipated coordination meetings, and deliverables such as reports required under the TOR (please use the form attached hereto as Annex F); 8) Technical Proposal (please use the form attached hereto as Annex G); and E. Financial Proposal: 9) Financial Proposal (must be inclusive of all applicable government taxes and charges) (please use the form attached hereto as Annex H) Bidders shall enclose their original Eligibility Documents and Technical Documents in one sealed envelope marked as “ORIGINAL – ELIGIBILITY AND TECHNICAL DOCUMENTS”, and the original of their Financial Proposal in another sealed envelope marked as “ORIGINAL – FINANCIAL PROPOSAL”, sealing them all by an outer envelope marked as “ORIGINAL PROPOSAL”. Each copy of the first and second envelopes shall be similarly sealed duly marking the inner envelopes as “COPY NO. ___ – ELIGIBILITY AND TECHNICAL DOCUMENTS” and “COPY NO. ___ - FINANCIAL PROPOSAL” and the outer envelope marked as “COPY NO. ___”, respectively. These envelopes containing the original and the copies shall then be enclosed in one single envelope. 4. Proposal shall be valid for a period of one hundred twenty (120) calendar days from the date of opening. 5. Proposal shall be evaluated using the Quality-Cost Based Selection/Evaluation (QCBS/E) procedure at a weight ratio of 80% for the Technical Proposal and 20% for the Financial Proposal. The criteria and rating system for the evaluation of the Technical Proposal are hereunder provided. The bid shall be rejected if it does not respond to the important aspect of the TOR or if it fails to achieve the required minimum score for Technical Proposal as hereunder set. CRITERIA RATING SCORE Quality of personnel to be assigned to the Project which covers suitability of key staff to perform the duties for the Project and general qualifications and competence including education and training of the key staff; 40 points Experience and capability of the Consultant which include records of previous engagement and quality of performance in similar and in other projects; relationship with previous and current clients; and, overall work commitments, geographical distribution of current/impending projects and attention to be given by the consultant. The suitability of the Consultant to the Project shall consider both the overall experiences of the firm and the individual experiences of the principal and key staff including the times when employed by other consultants; and 40 points Plan of approach and methodology with emphasis on the clarity, feasibility, innovativeness and comprehensiveness of the plan approach, and the quality of the interpretation of project problems, risks, and suggested solutions. 20 points TOTAL 100 points Note: The minimum score for Technical Proposal in order to qualify is 75 points. The Technical Proposal together with the Financial Proposal shall be considered in the evaluation. The Technical Proposal shall be evaluated first. The Financial Proposal of the Consultant who meets the minimum score for the Technical Proposal shall then be opened. 6. The complete set of Request for Proposal and TOR may be acquired by interested Consultants at the NHA-Region XII Office, Prime Regional Center, Brgy. Carpenter Hill, City of Koronadal, South Cotabato. It may also be downloaded free of charge from Philippine Government Electronic Procurement System (PhilGEPS) and NHA websites. 7. The NHA-Region XII reserves the right to accept or reject any and all bids, declare a failure of bidding, or not award the contract at any time prior to the contract award in accordance with Section 41 of RA 9184 and its IRR, without thereby incurring any liability to the affected bidder or bidders. 8.
