Statement of the prospective bidder’s of all its completed government and private contracts for the past five (5) years which is similar and not in nature and complexity to the contract to be bid (Form ER-1). A copy of the Certificate of Completion/Acceptance, from past clients or copy of Invoice(s) issued to the previous clients showing the payment made for the service(s) rendered, should be attached (refer to the provided Form ER-1); 2. Summary of On-going Contracts (Form ER-2) - may contain similar or not in nature and complexity of the project (refer to the provided Form ER-2); 3. Summary of Contracts Awarded But Not Yet Started, if any (Form ER-3) - may contain similar or not in nature and complexity of the project (refer to the provided Form ER-3); 4. The prospective bidder’s computation of Net Financial Contracting Capacity (NFCC) (Use the prescribed Form ER-4 provided), or a committed Line of Credit from a Universal or Commercial Bank in lieu of its NFCC computation; 5. Original duly signed and notarized Omnibus Sworn Statement (OSS) (Use the prescribed form); and if applicable, Original Notarized Secretary’s Certificate in case of a corporation, partnership, or cooperative; or Original Special Power of Attorney of all members of the joint venture giving full power and authority to its officer to sign the OSS and do acts to represent the Bidder. 2.
