Buwan-buwan St., Phase 1, Dagat-Dagatan, Navotas City Approved Budget for the Contract (ABC) Seven Hundred Forty Four Thousand Three Hundred Ninety Eight Pesos and Seventy Five Centavos Only (Php 744,398.75) Source of Fund Corporate Receipts Contract Duration Fifteen (15) Calendar Days Upon Receipt of Purchase Order Interested Office Equipment Provider/s are required to submit the following documents: 1. Mayor’s/Business Permit; 2. PhilGEPS Registration Number; 3. DTI Registration or SEC Certificate of Registration; and 4. if the RFQ and contract documents will be submitted/signed by an authorized representative: ● If Sole Proprietorship - Special Power of Attorney (See Form SPA) ● If Partnership, Corporation, Cooperative, or Joint Venture - duly notarized Secretary’s Certificate, Board/Partnership Resolution, or Special Power of Attorney, whichever is applicable. 5.
