Has experience in crafting of facility process master plan; 2. Has experience in conducting a SSOP writing and cGMP training; 3. Knowledgeable of the regulatory requirements for FDA-LTO licensing 4. Demonstrates professional resource person skills, 5. If the entity is a sole proprietorship, he/she must be registered with DTI, has Philgeps certification and has paid his/her professional tax. 6. If the entity is a group or organization, they are required to submit the name and CV of the consultant they intend to delegate, must be a duly established and registered organization in the Philippines with at least three years in existence. 7. The consultant delegated by the firm must hold a Food Safety Compliance Officer certificate or any other internationally recognized equivalent and preferably a graduate of any food-related programs. Deliverables: Capacitate ten (10) coconut industry players from Iligan City and the municipalities of Magsaysay, Tubod, Lala, Kapatagan and Sultan Naga Dimaropo with exceptional experience and knowledge during the crafting of the facility process flow master plan, seminar on cGMP, and writeshop on SSOP Manuals (Please see attached Terms of Reference for complete details of work) Additional Requirements: 1. The bidders shall submit the following Eligibility Documents in addition to the prescribed quotation form: (i) Registration certificate from the Securities and Exchange Commission (SEC) for corporations, Department of Trade and Industry (DTI) for sole proprietorship, or Cooperative Development Authority (CDA) for cooperatives, whichever is applicable. (ii) Latest Professional Tax Receipt (PTR) issued by the city or municipality where the principal place of business of the prospective bidder is located. (iii) Philgeps Certificate of Registration. (iv) Latest Income Tax Return (v) Omnibus Sworn Statement 2) All entries shall be typed or written in a clearly legible manner. 3) Prices are inclusive of taxes, delivery of items, and other incidental expenses.
