Contract Duration/ Delivery Date: List of Requirements: -Quotation Form -Copy of latest Mayor's Permit/Business Permit - Notarized Omnibus Sworn Statement (must be submitted only by winning supplier prior to award) -Copy of the latest Income Tax Return stamped received by BIR (first 2 pages only) - Site Inspection Certificate -Other requirements indicated in the Terms and Conditions •Proof of license and registered contractor/service provider by the Fertilizer and Pesticide Authority (FPA). •At least five (5) years experience in providing pest and termite control services to a government agency (excluding AFAB). Provide documents as proof such as business registration, purchase order, work order or equivalent. •Must provide certificate of satisfaction or client/customer feedback from with at least satisfactory rating from at least two (2) clients (at least one (1) from government agency) with whom the Service Provider has the same project within three (3) years (November 2021 until November 2024) •All technicians/personnel who will conduct pest control and treatment services must have undergone trainings conducted by the qualified trainers such as Pest Control Association of the Philippines or Philippine Federation of Pest Management Operator’s Association. (send proof of trainings) •The Service Provider shall provide the AFAB with the Safety Data Sheet of the products to be used. The detailed list of chemicals to be used must be approved by the Food and Drug Administration ( FDA). This shall be submitted with the bid quotation. Note: -The quote shall be deemed to include the cost of all taxes, duties, fees, levies, and other charges imposed under the applicable laws.
