The Jose B. Lingad Memorial General Hospital (JBLMGH), through the Internally Generated Fund intends to apply the sum of One Million Five Hundred Ninety-One Thousand Ten Pesos and 56/100 (PhP 1,591,010.56) being the Approved Budget for the Contract (ABC) to payments under the contract for Improvement of Central Laboratory at 2nd Floor Frontline Building under IB No. 2024-03-26 (06). Bids received in excess of the ABC shall be automatically rejected at bid opening. 2. The JBLMGH now invites bids for the above Procurement Project. Completion of the Works is required within One Hundred Fifty (150) Calendar Days. Bidders should have completed a contract similar to the Project. The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II (Instructions to Bidders). 3. Bidding will be conducted through open competitive bidding procedures using non-discretionary “pass/fail” criterion as specified in the 2016 revised Implementing Rules and Regulations (IRR) of Republic Act (RA) No. 9184. 4. Interested bidders may obtain further information from JBLMGH and inspect the Bidding Documents at the address given below from 8:00AM – 5:00PM, Monday to Friday. 5. A complete set of Bidding Documents may be acquired by interested bidders on March 06, 2024 to March 26, 2024 from given address and website/s below and upon payment of the applicable fee for the Bidding Documents, pursuant to the latest Guidelines issued by the GPPB, in the amount of Five Thousand Pesos Only (PhP 5,000.00); Cash or Cashier’s/Manager’s Check Payment. The Procuring Entity shall allow the bidder to present its proof of payment for the fees by attaching a photocopy of Official Receipt (OR) on Financial Documents during the submission of Bids. 6. The JBLMGH will hold a Pre-Bid Conference the March 14, 2024; 9:00AM through video conferencing/webcasting via “Microsoft Teams” (The link will be provided in the confirmation message after submission of registration in Google Form https://forms.gle/pwwTNsFWrbLNWZH27), which shall be open to prospective bidders. All participants must confirm their attendance by filling out the online registration form mentioned. In relation thereto, upon entering the Microsoft Teams room, the account must be renamed with the participant’s name and agency or company name. Only one (1) representative shall be allowed to attend the Pre-Bid Conference. 7. Bids must be duly received by the BAC Secretariat through manual submission at the address stated below for the bid opening on or before March 26, 2024; 1:00PM. Late bids shall not be accepted. 8. All bids must be accompanied by a bid security in any of the acceptable forms and in the amount stated in ITB Clause 15. 9. Bid opening shall be on March 26, 2024; 1:00PM at the Auditorium, 5th Floor, Surgery Building, JBLMGH, Dolores, City of San Fernando, Pampanga. Bids will be opened in the presence of the prospective bidders’ representatives who choose to attend the activity. Only one (1) representative shall be allowed to attend the Opening of Bids and must present an authorization letter approved by the duly authorized office of the company being represented. 10. The JBLMGH reserves the right to reject any and all bids, declare a failure of bidding, or not award the contract at any time prior to contract award in accordance with Sections 35.6 and 41 of the 2016 revised Implementing Rules and Regulations (IRR) of RA No. 9184, without thereby incurring any liability to the affected bidder or bidders. 11.
