Submission of proposals including Mayor’s/Business Permit*, PhilGEPS Registration Number*, and Notarized Omnibus Sworn Statement (OSS)**, addressed to Dr. Melvin B. Carlos through the BAC Secretariat shall be on or before October 17, 2024, at the physical address or through the email address below. All prices shall be denominated in Philippine Peso and quotation shall be firm and valid for a period of at least sixty (60) days from the date of receipt of the proposal. It shall be binding upon the supplier within the said period. The items shall be delivered within thirty (30) calendar days after receipt of the contract. The goods shall be awarded per lot.
