The Department of Education, Schools Division of Parañaque through MOOE Fund, intends to apply the sum of NINETY-FOUR THOUSAND AND FOUR HUNDRED PESOS ONLY (PHP 94,400.00) being the Approved Budget for the Contract (ABC) to payments under the contracts for Physical Examination of SDO Employees. 2. The SDO Paranaque through its Bids and Awards Committee (BAC) now invites Request for Quotation (RFQ) for the above-mentioned services, within seven (7) calendar days. Bidder shall submit fully sealed and duly signed by you or your authorized representative to the Bids and Awards Committee or its Secretariat not later than October 31, 2024, 11:30 a.m at DepEd Schools Division of Parañaque City. 3. Prospective bidders should have experience in undertaking a similar project within the last two (2) years with an amount of at least 50% of the proposed project for bidding. 4. Opening of Sealed Quotation is scheduled on October 31, 2024, 1:00 p.m, Jaime Ferrer Hall, DepEd Schools Division of Parañaque, Kabihasnan St. San Dionisio, Parañaque City and is restricted to Filipino citizens/sole proprietorship, partnerships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines, and to citizens, pursuant to RA 5183 and subject to Commonwealth Act 138. 5.
