Project: Production of Star Rating Plaques for Star-Rated Hotels, Resorts and Apartment Hotels (HRAs) II. Project Description: The project will entail the service of the winning bidder for the production of Star Rating Plaques that will be awarded by the OTSR-SMED to Star-Rated Hotels, Resorts and Apartment Hotels (HRAs) that have passed and fully complied with the criteria of the DOT’s National Accommodation Standards (NAS). III. Minimum Requirements for Bidders 1. Bidders must be PhilGEPS registered 2. Must be willing to provide product warranty/return and exchange of defective items 3. Must not have a history of cancellation of award/s or non-compliance with the deliverables based on the Procurement Division’s previous records/procurement. 4. Can deliver items on time. 5. Must be willing to facilitate complete/full delivery of finished products to the DOT Central Office and package/label accordingly. 6. Must be willing to provide services on a send-bill arrangement IV. Scope of Work/Deliverables The services required by the DOT End-user: 1. Submission of one (1) mock-up sample of plaque before mass production. 2. Proposed materials to be used as specified in the layout/description 3. Production and delivery of the following: V. Deliverables SPECIFICATIONS (Please see annex A for sample design/layout of star rating plaque) Quantity: 27 PCS. Plaque: Size 5” W x 7” H Plaque: Material (Surface) Etched Brass Matte Finish with Gloss Clear Coat (Non-tarnish Brass) Frame: Size 7.5” W x 9.5” H Frame: Material Capiz Shell (to submit sample design/s of capiz frame for approval of the End-user) Distinct designs: DOT Quality Seal – embossed; full color (size: 1.5”x 1.5”) *End-user will provide the high resolution DOT Quality Seal photo. Stars – embossed; Star casting with Gabled Points, Gold-tone finish (size: 0.80” x 0.80”) Border & Texts – Black engraved Text fonts – Sans Serif Back features: With frame stand; and Frame hanger Packaging: Hard box with DOT logo foil print gold in the middle VI. Budget Eighty-Nine Thousand One Hundred Pesos Only (Php 89,100.00) covering all products, services and other related expenses including taxes and other fees. VII. Schedule of Delivery/Timeline • Submission of sample within seven (7) working days upon receipt of approved Purchase Order. • Complete delivery of DOT plaques shall be made within 30 working days upon approval of the final design/sample. VIII. Contract Period: From approval of contract to completion of required quantity. IX. Delivery Address: The Delivery Address for the items will be at the: New DOT Building, \ 351 Sen. Gil Puyat Ave., 1200 Makati City, Philippines X. Terms of payment: Full payment shall be processed after delivery and acceptance of the plaques and upon receipt of the billing statement subject to Government accounting and auditing rules and regulations. XI. Compliance to Specifications/Provisions: • Non-submission of required mock-up sample and proposed materials to be used shall be grounds for disqualification of bid. • The WINNING BIDDER, shall ensure that the plaques are made in compliance with the given design and contents. Should there be minimal revisions to be made regarding the design and materials to be used, the supplier must coordinate with the end user prior to the finalization of the plaque. • The WINNING BIDDER shall warrant its performance in accordance with the specifications as stated in this TOR, and design/concepts as approved by the DOT-END USER and that any and all resulting defects or deviations shall be corrected by the WINNING BIDDER at its own expense immediately upon being informed thereof by the DOT-END USER. • The WINNING BIDDER shall hold in confidence all information which comes to their knowledge and shall not use, reproduce, nor disclose to others the approved DOT Plaque except to those persons entrusted/authorized by the DOT-END USER.
