Professional License/s as Real Estate Appraisal/Curriculum Vitae of the Licensed appraiser f. Certificates on CPD Seminar/Trainings attended within the last three (3) years), g. Statement of all its COMPLETED government and private contracts, whether similar or not similar in nature and complexity to the contract to bid, within the last five (5) years using Form SF-CONS-15A (Annex “B”); h. Statement of all its ON-GOING government and private contracts, including contracts awarded but not yet started, whether similar or not similar in nature and complexity to the contract to be bid, if any, using Form SF-CONS-15B (Annex “B”); 6. Proposals must be duly received by the CBAC Secretariat through manual submission at the Conference Area, CBAC Secretariat’s Office, 2nd floor Annex Building, NHA Main Compound, Elliptical Road, Diliman, Quezon City on July 22, 2024 not later than 9:30 AM. All Proposals must be accompanied by eligibility documents as required. Late submission of proposals shall not be accepted. 7.
