Certified copy of License to Operate issued by FDA or appropriate agency (for drugs and meds/hospital& lab supplies/chemical products) 4. Certified copy of Certificate of Product Registration issued by FDA or appropriate agency, (required for drugs and medicines and chemical products; for medical devices, as applicable per FDA Memo Circular 2014-005) 5.
