Delivery period is within Ten (10) calendar days from receipt of Notice to Proceed. 9. Bidders must submit/attach legal documents pursuant to the Revised Implementing Rules and Regulations of RA 9184 (Latest Edition) upon submission of Quotation. PhilGEPS Registration Number Mayor's/ Business Permit To be submitted upon signing of Purchase Order: Original Notarized Omnibus Sworn Statement and if applicable, Original Notarized Secretary's Certificate in case of corporation, partnership, or cooperative, or original SPA giving full power and authority to its officer to sign the OSS and do acts to represent the supplier/ contractor. 10. Bids submitted thru Mail, Email or Fax shall not be accepted. The procuring entity reserves the right to reject outright any contingent bid/ offer/ quotation. 11.
