Delgado Streets, Port Area Manila. The following documents are also required to be submitted along with your quotation on the specified deadline above: Requirements Instructions PhilGEPS Registration Number Submit Proof of Registration (picture or screenshot of PhilGEPS Registration on the website) Mayor’s or Business Permit In case not yet available, you may submit your recently expired Mayor’s or Business Permit with the Official Receipt of renewal application. However, a copy of your updated and valid Mayor’s or Business Permit shall be required to be submitted during post-qualification. Income/ Business Tax Return Latest Annual Income Tax Return or Business Tax Return covering the previous six (6) months PCAB License License Category: Trade E Original Notarized Omnibus Sworn Statement (OSS), and if applicable, Original Notarized Secretary’s Certificate (SC) in case of a corporation, partnership, or cooperative; or Original Special Power of Attorney (SPA) of all members of the joint venture giving full power and authority to its officer to sign the OSS and do acts to represent the Bidder.
