Roxas Blvd., Pasay City Approved Budget for the Contract (ABC): One Hundred Thousand Pesos Only (₱100,000.00) Source of Fund: MOOE Contract Duration: Five (5) working days upon receipt of Purchase Order (P.O.) Interested Supplier/s are required to submit the following documents together with the accomplished form (Annexes A, B, and C): a) Mayor’s/ Business Permit b) PhilGEPS Registration Number c) Omnibus Sworn Statement (Notarized) Award of contract shall be made to the lowest calculated and responsive quotation, which complies with the Item Description. The Supplier’s offer or quote above the Approved Budget of the Contract (ABC) shall be automatically disqualified.
