As such, you are invited to submit your establishment’s quotations/proposal duly signed by you or your duly authorized representative not later than May 22, 2024 at 1:00 o’clock in the afternoon for the item described below, subject to the General Conditions provided at the dorsal portion of this request for quotation (RFQ). Copies of the following eligibility requirements are also required to be submitted along with your quotation/proposal: 1. Business Permit 2. Electronically Filed Income or Business Tax Return [Filed through the BIR Electronic Filing and Payment System (eFPS) or Annual Income or Business Tax Return 3. PhilGEPS Certificate of Registration or Registration No. 4.
