Reyes (Busuanga) Airport, thru its Canvass and Contract Committee (FBRA-CCC), will undertake a Small Value Procurement (SVP) for the “Catering Service and Venue Rental for Team Building Activity CY2024” in accordance with Section 53.9 of the Implementing Rules and Regulations of Republic Act No. 9184. RFQ No. : 2024-09-027 Date : September 9, 2024 Approved Budget for Contract: Php 62,096.00 Delivery Period : September 14-15, 2024 on or before 8:00AM Interested suppliers are required to submit their quotation using the attached Price Quotation Form (Annex A) address to the undersigned in a sealed envelope or send thru FBRA-CCC official e-mail bac.caapfbra@gmail.com on or before 2:00PM of September 12, 2024 with the following valid and current documents: 1. Mayor’s or Business Permit issued by the city or the municipality; 2. PHILGEPS Registration Number; 3. Notarized Omnibus Sworn Statement (GPBB prescribed form) for ABC’s above 50,000; 4. Special Power of Attorney of Authorized Representative for Sole Proprietorship and Secretary Certificate for Corporation; and 5. Proposed Menu for Snacks and Buffet Lunch. Price quotation/s must be valid for a period of ninety (90) calendar days from the date of submission. Quotations exceeding the Approved Budget for Contact shall be rejected. Award of contract shall be made to the lowest quotation, which complies with minimum description as stated above and other terms and conditions stated in the price quotation form. In case two or more bidders are determined to have submitted the Lowest Calculated and Responsive Quotation, FBRA-CCC shall adopt and employ “draw lots” as the tie breaking method to finally determine the single winning provider in accordance with GPPB Circular 06-005. Any interlineations, erasures or overwriting shall be valid only if they are signed or initialed by the bidder or his/her duly authorized representative/s. Liquidated damages equivalent to one tenth of the ten percent (0.1%) of the value of goods not delivered within the prescribed delivery period shall be imposed per day of delay. CAAP-FBRA shall rescind the contract once the cumulative amount of liquidated damaged reached ten percent (10%) of the amount of the contract, without prejudice to other courses of action and remedies open to it.
