Name of Project: Supply and Delivery of Construction Materials for the Installation of Solar Panel Street Lights (18 units) at Barangay Nungnungan I, Cauayan City, Isabela Approved Budget: One Hundred Ninety Eight Thousand One Hundred Ninety for the Contract Pesos and Fifty Centavos (P198,190.50) Specifications: See the attached Annex “A” for specifications Location: Cauayan City, Isabela Delivery Date: Within Seven (7) calendar days from receipt of Purchase Order Interested suppliers are required to submit two (2) copies of their valid and current Mayor’s Permit, Income/Business Tax Return for ABC’s above P500,000, PhilGEPS Registration Number, Omnibus Sworn Statement and price quotation form (Annex “A”) during submission of offer/quotation. Award of contract shall be made to the lowest quotation, which complies with the minimum description as stated above and other terms and conditions stated in the price quotation form. Any interlineations, erasures or overwriting shall be valid only if they are signed or initialed by the bidder or his/her duly authorized representative/s.
