RATIONALE To enhance the operations and delivery of frontline services in order to gain public trust and confidence as well as pursuant to Executive Order no. 605 series of 2007 signed by former President Gloria Macapagal – Arroyo on 23 February 2007 institutionalizing the structure, mechanisms and standards to implement the Government Quality Management Program in all public sector organizations, the LGU – San Agustin desires to be ISO 9005:2015 Certified in year 2025. The QMS will improve the municipality’s performance by ensuring the consistency of products and services through quality processes. This will also serve as an effective tool in satisfying customers’ expectations through excellent provision of public service. The objective of ISO 9001:2015 criteria is to enhance the organization's worth. In order for the organization to actually enjoy the benefits that have been promised, they must be adequately directed in the establishment and upkeep of a quality management system. The implementation of a Quality Management System (QMS) can yield significant benefits for the Municipal Government, including increased client satisfaction, streamlined and efficient procedures, and the establishment of a framework for ongoing improvement. Thus, the Local Government Unit (LGU) of San Agustin, Romblon is looking for consultants to help with the Quality Management System (ISO 9001:2015) and to facilitate its certification. This will enable them to get ready for the QMS requirements that are in line with the International Organization for Standardization (ISO) 9001:2015 Standard and eventually get certified by an international certification body. II. OBJECTIVE The primary objective of the project is to hire the professional services of accredited consultant to establish and implement the clauses of ISO 9001:2015 Standard. The role of the consultant throughout the project is to provide and transfer expert knowledge. This will be achieved through trainings, workshops, guidance in reviewing the developed policies and necessary documents, guidance in implementing the QMS and the conduct of pre-certification audit. These include the following: 1. Establishes Quality Management System in the most effective, economical, and timely manner with all relevant considerations and constraints taken into account; 2. Designs and develops a risk-based approach that identifies risk and opportunities within the LGU – San Agustin appropriate to the purpose and mandates of this Municipality; 3 Prepares all levels of staff of LGU – San Agustin for a successful implementation and effective long-term maintenance of an ISO 9001:2015 System; 4. Motivates employees towards the direction of improving the quality of test for cost effectiveness; 5. Prepares the required documentation – policies, manuals, documented procedures, work instructions, records – for the efficient and effective operation and for certification; 6. Provides support during the certification audit and in developing strategies and actions to address non-conformances and observations. III. SCOPE OF WORK AND DELIVERABLES The consultant shall engage to perform tasks specific to the Project entitled: Development and Implementation of a Quality Management System Compliant and Certifiable to ISO 9001:2015. The development of the Quality Management System (QMS) shall be for the alignment of the existing management system with ISO 9001:2015 standards leading to Certification of Local Government Unit (LGU) of San Agustin, Romblon. IV. OBLIGATIONS OF THE CONSULTANT 1. Organize a Team who will conduct the project within the agreed time frame; 2. Prepare the program and course designs for all the training and workshop activities; course to be conducted at least one (1) week prior to the actual training; 3. Provide resource persons/technical experts to conduct the training courses; 4. Provide certificates of participation or completion for qualified participants of training courses conducted; 5. Guide and assist in the drafting of the Quality Manual, Quality Policy, Quality Procedures, and Standard Operational Instructions, and recommend necessary revisions to ensure alignment of these documents with ISO 9001:2015 standards; 6. Provide technical assistance and guidance to the LGU – San Agustin ISO Core team in establishing and implementing the Quality Management System until ISO 9001:2015 certification; 7. Consultant shall perform the necessary tasks at NO ADDITIONAL COST up until the passing of the certification audit. V. MINIMUM QUALIFICATIONS OF THE CONSULTANT (to be used during eligibility check and short listing) The Consultant must meet the following qualifications: 1. (a) Duly licensed Filipino citizens/sole proprietorships; or (b) Partnerships duly organized under the laws of the Philippines and of which at least sixty percent (60%) of the interest belongs to citizens of the Philippines; or (c) Corporations duly organized under the laws of the Philippines and of which at least sixty percent (60%) of the outstanding capital stock belongs to citizens of the Philippines; or (d) Cooperatives duly organized under the laws of the Philippines, and of which at least sixty percent (60%) interest belongs to citizens of the Philippines; or (e) Persons/entities forming themselves into a joint venture, i.e., a group of two (2) or more persons/entities that intend to be jointly and severally responsible or liable for a particular contract: Provided, however, That Filipino ownership or interest thereof shall be at least sixty percent (60%). For this purpose, Filipino ownership or interest shall be based on the contributions of each of the members of the joint venture as specified in their JVA. 2. The Consultant must have a proven track record in assisting companies/agencies especially Local Government Units to develop, implement and get certification for ISO 9001:2015 quality management system for at least five years; 3. The Lead Consultant must be Certified and Affiliated with the International Register of Certified Auditors (IRCA) and CQI. VI. SELECTION CRITERIA FOR EVALUATION The LGU – San Agustin thru its Bids and Awards Committee (BAC) shall select the most qualified ISO 9001:2015 Consultant using the quality cost-based methodology pursuant to the pertinent provisions of R.A. No. 9184 wherein the Technical Proposal together with the Financial Proposal shall be considered. CRITERIA PERCENTAGE (%) 1. Technical Proposal a. REQUIRED SUBMISSIONS The consultants are required to submit the following documents: 1. Technical Proposal a. Statement of all ongoing and completed government and private contracts including contracts awarded but not yet started, if any. Please include Certificates of Completion for each completed project; b. List of professional staff who will conduct the project including their Curriculum Vitae stating therein their complete qualifications and experience data; c. Narrative Description of the Proposed Approach and Methodology 2. Financial Proposal a. Proposed amount for the professional consultancy services based on the activities set in the Terms of Reference. Pursuant to Annex H of the Revised IRR of RA 9184, the following requirements shall be submitted together with the technical and financial proposals: 1. Income or Business Tax Return; 2. PhilGEPS Registration Number; 3. BIR Certificate of Registration shall be submitted in lieu of DTI Registration and Mayor’s Permit (as required specific to individual consultants); 4. Omnibus Sworn Statement; and 5. Other documents stated under Consultant’s Qualification Requirements of this TOR. 3. Submission of Proposal The Proposals to be sent only in hard copy by post/courier/hand carry. Submission will be in three (3) copies marked original, first copy and second copy. The plan of approach and methodology shall be included in the technical proposal that shall be submitted by the Consultant/Bidder, in addition to the above documentary requirements. This contains the methods, approaches, and proposed implementation activities and the corresponding schedule consistent with the TOR. VIII. APPROVED BUDGET FOR CONTRACT The Approved Budget for the Contract (ABC) is Nine Hundred Ninety-Nine Thousand Seven Hundred Fifty pesos (P999,750.00) inclusive of VAT and shall be deemed to include the cost of all taxes, duties, fees, levies and other charges imposed under applicable laws. IX. PAYMENT SCHEME/SCHEDULE Payment for the services rendered of the selected consultant shall be in accordance with the following schedule as percentage (%) of contract price. LGU – San Agustin, Romblon shall provide the logistical requirements for the conduct of the trainings, workshops and consultations, and payment of all the associated costs, including training venue, meals, traveling expenses and room/hotel accommodation, if needed. X. LGU – San Agustin, Romblon reserves the right to reject any or all proposals, or to waive any defect or informality thereon or minor deviations, which do not affect the substance and validity of any or all of the proposals. XI. LGU – San Agustin, Romblon reserves the right to reject the proposal of any proponent who: a. does not offer the required services as provided for in this Terms of Reference; b. is discovered to have suppressed, disclosed or falsified information; or c. failed to satisfactorily perform/complete any contract previously awarded to it. XII. LGU – San Agustin, Romblon reserves the right to review other relevant information affecting the proponent or the proposal before the approval of the contract. Should such review uncover any misrepresentation made in the proposal documents, or any change in the situation of the proponent which affects the substance of the proposals, LGU – San Agustin may disqualify the proponent from obtaining the award.
