In view of the two (2) failed biddings, the Local Government of San Isidro, Surigao del Norte Bids and Awards Committee now invites interested supplier to participate in the negotiation for the Supply and Delivery of assorted 3rd and 4th quarter medicines to apply the sum of One Hundred Eleven Thousand Sixty Pesos & 04/100 (Php111,060.04) being the Approved Budget for the Contract(ABC) to payments under the contract for the “Supply and Delivery of assorted 3rd and 4th quarter medicines in accordance with section 53.1 of the Implementing Rules and Regulations of Republic Act No. 9184, otherwise known as the “Government Procurement Reform Act”. 2. The following eligibility and technical documents as well as the Financial Proposals Form (Annex A). shall be submitted on or before November 4, 2024 at 1:30pm at the Mayor’s office, Conference Room, Municipal Hall, San Isidro, Surigao del Norte. a. Department of Trade and Industry Registration Certificate, b. Valid Mayor’s Permit, c. Request for Quotation (RFQ) d. Omnibus Sworn Statement upon request for payments. e. PhilGEPS Registration. 3. Opening of eligibility documents, technical documents and financial proposals will be on November 4, 2024 at 2:00pm 4.
