A copy of the Certificate of Completion/Acceptance, from past clients or copy of Invoice(s) issued to the previous clients showing the payment made for the service(s) rendered, should be attached (refer to the provided Form ER-1); 2. Summary of On-going Contracts (Form ER-2) - may contain similar or not in nature and complexity of the project (refer to the provided Form ER-2); 3. Summary of Contracts Awarded But Not Yet Started, if any (Form ER-3) - may contain similar or not in nature and complexity of the project (refer to the provided Form ER-3); 4. The prospective bidder’s computation of Net Financial Contracting Capacity (NFCC) (Use the prescribed Form ER-4 provided), In case of Joint Venture, the submission of technical and financial eligibility documents by any of the JV partners constitutes compliance, provided, that the partner responsible to submit the NFCC shall likewise submit the following: i. Statement of all of its on-going government and private contracts, including contracts awarded but not yet started; and ii. Audited Financial Statements 5. Valid Philippine Contractors Accreditation Board (PCAB) License of at least category “E” under SP-Trade; or Special PCAB License in case of Joint Ventures; and registration for the type and cost of the contract to be bid; 6. Project Requirements, which shall include the following: 6.1) Organizational chart for the contract to be bid; 6.2) List of contractor’s key personnel (e.g., Project Manager, Project Engineers, Materials Engineers, and Foremen), to be assigned to the contract to be bid, with their complete qualification and experience data (Please refer to the Terms of Reference item No. 7 – Manpower Requirements, for complete details); 6.3) List of contractor’s major equipment units, which are owned, leased, and/or under purchase agreements, supported by proof of ownership or certification of availability of equipment from the equipment lessor/vendor for the duration of the project, as the case may be; 7. Original duly signed and notarized Omnibus Sworn Statement (OSS) (Use the prescribed form); and if applicable, Original Notarized Secretary’s Certificate in case of a corporation, partnership, or cooperative; or Original Special Power of Attorney of all members of the joint venture giving full power and authority to its officer to sign the OSS and do acts to represent the Bidder. 2.
