Peruukit (Apuvälineet) Hankinnan kohteena on HUS Yhtymän lääkinnällisen kuntoutuksen apuvälineenä maksusitoumuksella hankkimat peruukit. Peruukit hankitaan yleensä ihosairaus- ja/tai syöpäpotilaalle, jolla on laaja-alaista, tilapäistä tai pysyvää hiustenlähtöä sairaudesta tai hoidoista johtuen. Lääkinnällisenä kuntoutuksena myönnettävien peruukkien apuvälinepalveluprosessi on osa asiakkaan kuntoutus- ja hoitoprosessia, ja sen tavoitteena on löytää potilaan yksilöllistä tarvetta vastaava, tavanomaiseen arkikäyttöön soveltuva peruukki, joka on valittu yhdessä asiakkaan kanssa. Lääkinnällisen kuntoutuksen apuvälineenä peruukki on saajalleen maksuton. Syöpä ja/tai ihotautien erikoislääkäri arvioi yksilöllisesti asiakkaan peruukkitarpeen ja muut mahdolliset erityistarpeet ja kirjoittaa maksusitoumuslausunnon. Maksusitoumus myönnetään ennen peruukin ostoa ja se tulee olla potilaalla mukana peruukkia hankittaessa. Hankinnan kohde on jaettu osa-alueisiin: - Osa-alue 1: Vakiomallinen peruukki - Maksusitoumusluokka A, maksimihinta 160 euroa (alv 0%) - Osa-alue 2: Vakiomallinen peruukki pehmeällä pohjalla - Maksusitoumusluokka B, maksimihinta 350 euroa (alv 0%) - Osa alue 3: Yksilöllinen, erityistarpeen peruukki pehmeällä pohjalla ja kuumuutta kestävällä hiusmateriaalilla - Maksusitoumusluokka C, maksimihinta 500 euroa (alv 0%) - Osa-alue 4: Yksilöllinen, asiakkaalle muotoiltu ja istuva erityistarpeen peruukki - Maksusitoumusluokka D, maksimihinta 950 euroa (alv 0%) - Osa-alue 5: Päähineet ja huivit, hiuslisäkkeellä tai ilman - Maksusitoumusluokka E, maksimihinta 45 euroa (alv 0%) - Osa-alue 6: Pehmopohjaperuukit erityisen herkälle iholle (ilman verkkomaista etuosaa / tylliä) - Osa-alue 7: Ensisijaisesti miehille suunnatut peruukit Tarjoaja voi tarjota yhtä (1) tai useampaa osa-aluetta. Lisäksi tuoteryhmät 6 ja 7 on jaettu edelleen neljään (4) tuotekoriin (6.1 - 6.4 ja 7.1-7.4), jotka muodostavat kilpailutuksessa omat osa-alueensa. Tarjoaja voi tarjota osa-alueissa 6 ja 7 yhtä (1) tai useampaa tuotekoria. HUS valitsee osa-aluekohtaisesti enintään kolme (3) sopimuskumppania per osa-alue. Sopimuskausi on 3 vuotta. Sitä voidaan jatkaa yhden (1) vuoden mittaisella optiokaudella. Hankinnan ennakoitu arvo on noin 290 000,00 euroa / vuosi, eli yhteensä 1 160 000 euroa (laskettuna neljälle (4) vuodelle) Kyse on ainoastaan arviosta, eikä HUS sitoudu mihinkään tilausmääriin. Lisäksi hankinta sisältää option siitä, että Asiakas (peruukin käyttäjä) voi halutessaan hankkia omalla kustannuksellaan maksusitoumuksen euromäärää kalliimman peruukkimallin (ts. sopimusvalikoimasta poikkeavan peruukin), maksamalla itse peruukin ja maksusitoumuksen välisen summan erotus. Näistä optiotuotteista pyydetään liittämään erilliset hinnastot tarjouksen yhteyteen. TILAUSTEN JAKAUTUMINEN SOPIMUSKAUDELLA Lääkinnällisenä kuntoutuksena myönnettävien peruukkien apuvälinepalveluprosessi on osa Asiakkaan kuntoutus- ja hoitoprosessia, ja sen tavoitteena on löytää potilaan yksilöllistä tarvetta vastaava, tavanomaiseen käyttöön soveltuva peruukki, joka on valittu yhdessä Asiakkaan kanssa. Peruukkipalvelu on aina yksilöllistä asiakaspalvelua, jossa peruukki mitataan ja sovitetaan käyttäjälle henkilökohtaisesti. Syöpä- ja/tai ihotautien erikoislääkäri arvio asiakaskohtaisesti Asiakkaan peruukkitarpeen ja muut mahdolliset erityistarpeet ja kirjoittaa maksusitoumuslausunnon. Maksusitoumus myönnetään ennen peruukin hankintaa ja se tulee olla Asiakkaalla mukana peruukkia hankittaessa. Asiakas valitsee itse HUS sopimustoimittajista tarvettaan parhaiten vastaavan palveluntuottajan. The payment commitment is issued before the purchase of the wig and it must be with the Customer when purchasing the wig. The customer himself chooses the service provider that best meets his needs from HUS contract suppliers.
