Quotations must be submitted on or before 09:00 AM 24 May 2024 (Friday) at PAFPC Conference Room, Col Jesus Villamor Air Base, Pasay City. All quotations must be accompanied by eligibility documents and for the negotiated 53.9 attached Omnibus Sworn Statement for the project above 50K and Income/Business Tax Return for the project 500K above as required. (Per project prepare 2 folders with tabbing’s: Tab A-Eligibility Requirements and Tab B- Request for Quotation (RFQ) with original signature and put in envelope and sealed with signature). 3. RFQ will be opened in the presence of the Suppliers or their representative/s who chooses to attend at the address stated above. Late quotations shall not be accepted. 4.
