Name of the Project : Repair and Maintenance of Public Cemetery Location : Pagbilao, Quezon Brief Description : Repair and Maintenance of Public Cemetery Approved Budget : P 52,994.48 Contract Duration : Thirty (30) days A complete set of quotation documents may be obtained by interested proponents on October 24 to October 28, 2024 from the BAC Secretariat at the Conference Room, Administration Building, Pagbilao, Quezon. Moreover, eligibility documents to be submitted are (i) photocopy of proponent’s DTI or Securities and Exchange Commission Registration Certificate, as the case maybe; (ii) Mayor’s Permit; and (iii) BIR Registration (iv)Philgeps Certificate or Registration and Membership; (v) Tax Clearance; (vi) Income Return Tax; (vii) Audited Financial Statement; and (viii) Omnibus Sworn Statement. Submission of eligibility and quotation documents is on or before October 28, 2024 9:15 a.m. at Conference Room, Administration Building while opening of said documents will follow thereafter at the same address indicated above. Late submission shall not be accepted. The forms, which are part of the quotation documents, shall be the same forms to be submitted by the proponents for their compliance with the schedule of requirements, technical specifications, and financial proposal submission sheet. All pages of the proposals shall be initialed by the person/s signing the Bid. Non-compliance therewith shall be a ground for disqualification. The “No Contract Rule” shall apply. Bidders are not allowed to call or talk to any member of the BAC, TWG, or Secretariat right after the opening of the eligibility and quotation documents. The proponent determined to have the Lowest Calculated Bid shall be required to furnish the BAC the Certificate of PhilGEPS Registration within three (3) calendar days from receipt of notice thereof.
