Valid Mayor’s/Business Permit¹ c. PhilGEPS Registration Number (Red Membership) or PhilGEPS Certificate (Platinum Membership) d. Professional License/Curriculum Vitae (for Consulting Services) e. Latest Annual Income / Business Tax Return (for ABCs above P500K) NOTE: Submitted documents are subject to verification and validation of its authenticity, genuineness, validity, and completeness. Incomplete submission, concealment, falsification, or misrepresentation of any of the documents submitted, or the contents thereof is a ground for disqualification of submitted quotation. ¹ For individuals engaged under Sec. 53.6, 53.7 and 53.9 of the IRR of RA No. 9184, only the BIR Certificate of Registration shall be submitted in lieu of DTI Registration and Mayor’s Permit.
